While we prefer you to submit the Check Request Form electronically, hard copies of the check
request form can be found in the PTA Treasurer folder in the school office.
Completed forms should be dropped off in the PTA Treasurer folder in the school.
Tax Exempt Letter

The PTA cannot reimburse you for any sales tax on items you purchase. The El Sierra PTA has a tax-exempt status with the state. This means that you will not have to pay sales tax when you are purchasing goods or services for El Sierra PTA program or fundraiser.
To utilize this for valid purchase, print or save a copy of our Tax Exempt Certificate and show it at time of purchase.

Complete a Check Request Form for any purchases. All committee chairs and PTA board members will have check request forms and tax exempt letters. The office has copies of both documents as well.
Please don’t pay the invoice with your own check or credit card unless time is of the essence.
All invoices from companies should first try to be paid via a PTA check.

Use your own credit card/cash and then submit the receipt for reimbursement by completing a Check Request Form.
Do not forget to use the Tax Exempt Certificate as the PTA does not reimburse for taxes.
You will normally get your reimbursement within 2 weeks.
